Submitting an appointment request does not guarantee an appointment. Your appointment is confirmed only when our office confirms the date and time.

Insurance coverage and benefits are determined by your insurer. Benefit estimates do not guarantee payment.

Where financing is offered, approval, interest rates, down payments, repayment terms, and other conditions depend on the financing provider and the applicable agreement. Advertised monthly payments do not guarantee approval or represent the total treatment cost.
